Primary Care Networks (PCNs) are groups of family physicians who work with other health care professionals like dieticians, social workers, nurses, pharmacists, and mental health workers to get their patients the best care possible.

PCN For Patients

A Primary Care Network (PCN) is a collaborative effort between family physicians, nurse practitioners, clinicians, and allied healthcare providers dedicated to improving patient healthcare experiences. Created by the BC Ministry of Health, our PCN is a partnership between the Shuswap North Okanagan Division of Family Practice, Interior Health, and Indigenous partner organizations. Our mission is to empower individuals of all ages with essential healthcare information, ensuring seamless collaboration between care providers for a comprehensive primary healthcare experience.

PCN For Physicians

The Shuswap North Okanagan PCN is a community-based network of healthcare professionals working together to improve access to primary care. This includes Family Physicians, Nurse Practitioners, Registered Nurses, Physiotherapists, Social Workers, Dietitians, Pharmacists, and more, all collaborating to provide coordinated, patient-centered care.

Our PCN currently has four separate PCN nodes:
✔ Vernon PCN (Vernon, Lumby, Armstrong)
✔ Salmon Arm PCN (Salmon Arm, Sicamous, Sorrento, Enderby)
✔ Splatsin PCN
✔ Okanagan Indian Band PCN

Referral Information for Physicians​

Physicians can refer patients to PCN services such as mental health support, physiotherapy, chronic disease management, and more.
